Patient-Centered, Family-Focused Compassionate Care

Patient-Centered,
Family-Focused Compassionate Care

We believe the best kind of care is patient-centered. This means that the care we provide gets defined by the wishes and needs of patients and their families. Each patient and family experiences highly individualized care when they choose us. Avoiding unnecessary hospitalizations that cause stress and hardship and helping the patient remain wherever they call home is a big reason why we have the highest quality of care in Dallas and the surrounding communities.

Expert care relies on an expert team. That is why we give our experienced clinicians the time and resources they need to keep the promise of the highest quality care. Our support is not only focused on our patients and families but the amazing team that serves them. We are proud to be locally owned and operated, with a focus on bettering the lives of those in Dallas and the surrounding areas.

Home Care

Provided wherever our patients call home including private residences, assisted living, nursing homes, or residential facilities.

Visits are made to meet the medical, spiritual, and emotional needs of our patients and families.

Patient needs are met through scheduled and on-call visits up to 7 days a week

The comfort of our patients is our number one goal. Our supportive and thorough staffing model allows us to increase how often we visit when clinically necessary to keep our patients comfortable. The experience and visit frequency of our staff is second to none and allows us to manage symptoms and avoid any crises that may arise.

General Inpatient Care

When a patient’s symptoms cannot be medically managed at home, this temporary/short-term care is provided in a skilled nursing facility.
We always strive to keep our patients wherever they call home. We also contract with facilities when the need for inpatient stay is necessary.

Appropriateness for General Inpatient Care is evaluated daily by an RN.

Our goal is to get the patient’s condition stabilized so they can return home as soon as it is clinically appropriate.

Respite Care

Respite Care

Caring for someone with serious health issues can be a draining experience for the primary caregiver and other family members. We understand this and take it very seriously.
Respite Care gives a home patient’s family time to attend out-of-town events (graduation, wedding, etc.) or take a much-needed break to rest.
Trust our experienced clinical and spiritual care team to assist in finding appropriate respite care facilities.

Continuous Care

Temporary/short-term care is provided in the patient’s home when symptoms are not well controlled through normal visits.
Nurses or Home Health aides are staffed at the bedside for up to 24 hours a day. Appropriateness for Continuous Care is evaluated daily by an RN.

Continuous care nurses communicate with our physicians and RN Case Managers to address changes and manage the patients’ condition.

During a crisis, it is important to feel comfortable. Continuous Care provides that level of support to patients and families. Through bedside support, our staff maintains the comfort of the patient clinically and provides emotional support for the family and loved ones. Our team creates an environment where families can feel comfortable spending time with their loved one.
