Home Health Care Support

As a leading hospice health care provider offering services in the greater Dallas area, including Rockwall TX, we know that creating a comfortable environment makes a world of difference to our patients. While at home our staff will be able to administer medications, be provided health care supplies and medical equipment to maximize comfort. In situations where a patient is staying in a long-term care facility, or has a shorter stay with an inpatient facility we are able to provide the same comprehensive specialty care we would otherwise offer in their home. While it is always our goal to keep your loved one comfortable in their own home, there may be times when it becomes difficult to manage symptoms outside of a healthcare facility. In situations like this we will focus our efforts to manage symptoms effectively so we can continue our care services in the patient’s home.
Types of Hospice Services
Because every patient and situation is unique we offer a variety of hospice services, and can make adjustments as necessary. Our Routine Home Care services are for patients that require regularly scheduled visits in their home or other care facility, from anywhere between 1 and 7 days per week. Our team will help manage symptoms, administer medication, monitor changes in health, and more. Because our services are adaptive we are able to quickly make changes to our care plans and can increase or decrease the frequency of these scheduled visits to accommodate your loved one’s health. In the event a patient needs more intensive care than scheduled visits can offer we are able to increase care to a Continuous Care plan and provide our services 24 hours a day. While we monitor health changes with every visit, during Continuous Care we are able to observe the patient’s health constantly and make decisions without delay. In some cases Continuous Care is not necessary because the family provides sufficient part-time care for their loved one. When family members need to temporarily step away from caring for their loved one we offer Palliative Care, in which we can provide full-time care for an assigned period of time. Whether the family members may have an out of town obligation, other temporary responsibilities, or simply need to pause from care to rest, Palliative Care offers some unrestricted time away.
