Continuous Hospice Care
Continuous hospice care is provided when a patient is experiencing severe symptoms that require 24/7 care. Our team will provide care in the patient’s home for a minimum of eight hours per day to manage your loved one’s symptoms and provide support for your family. We typically provide continuous hospice care for a limited period of time until the patient’s symptoms are under control and can be managed with routine home care. Continuous care may also be used during a period of crisis or when the patient is nearing the end of their life.
Our Bereavement Services
Losing someone you love is one of the hardest things you can experience. Our hospice team provides up to 13 months of bereavement support to help you through this difficult time. We provide the following bereavement services for our patient’s family members:

Our team will send letters every few months and call you to see how you’re doing.
We offer a memorial service once a year to honor and remember your loved one.

Why High-Quality Hospice Care Matters
Improved Quality of Life
Hospice care focuses on making patients more comfortable. We provide personalized care plans to meet your loved one’s unique needs.
Hospice care providers also support and educate caregivers. We can help caregivers take better care of their loved ones and provide resources to help caregivers manage their own stress.
Hospice care involves managing a patient’s disease to make them as comfortable and pain-free as possible during their final days. Your loved one’s comfort is our number one priority.
Less Time in the Hospital
Quality hospice care helps patients avoid unnecessary time in the hospital. Our services make it possible for patients to live in the comfort of their own homes during their final days.
Emotional & Spiritual Support
Quality hospice care doesn’t just involve managing physical needs. We also provide emotional and spiritual support to help patients and their families through these difficult times.
Hospice care services can be less expensive than traditional end-of-life healthcare. Hospice care services are usually covered by Medicare and some private insurance plans.
