Palliative Care in Fort Worth, TX

Star City Hospice provides multiple levels of palliative care to meet your loved one’s needs. Our services are patient-centered, which means the care we provide is defined by you and your family. As a Medicare-certified hospice center, we offer four levels of palliative care to Fort Worth:

Routine Home Care

Routine home care is the most common type of palliative care. This type of care is provided to patients who are living at home or in a long-term care facility. Our team will manage your loved one’s symptoms, provide emotional and spiritual support, and offer education and resources to your family. Our routine home care team includes doctors, nurses, social workers, chaplains, and other specialists as needed. We meet our patients’ needs through scheduled and on-call visits up to 7 days a week.

General Inpatient Care

While we always strive to keep our patients at home, sometimes general inpatient care is necessary. General inpatient care is provided in a skilled nursing facility for patients who require more intensive short-term care to treat their condition. General inpatient care may be necessary if your loved one is experiencing severe symptoms that cannot be managed at home, like pain, nausea, or difficulty breathing. During this time, our palliative care team will work closely with your healthcare providers to manage symptoms and provide support.

Respite Care

Respite care is a level of hospice care that gives a patient’s primary caregiver a break from their caregiving duties. The patient is temporarily placed in a hospice facility, hospital, or long-term care facility, where they will receive 24-hour care and support. We usually provide respite care for up to five days at a time. Our teams can provide respite care on a periodic or as-needed basis to give caregivers a chance to rest and recharge.

Continuous Hospice Care

Continuous hospice care is provided when a patient is experiencing severe symptoms that require 24/7 care. Our team will provide care in the patient’s home for a minimum of eight hours per day to manage your loved one’s symptoms and provide support for your family. We typically provide continuous hospice care for a limited period of time until the patient’s symptoms are under control and can be managed with routine home care. Continuous care may also be used during a period of crisis or when the patient is nearing the end of their life.

Our team will send letters every few months and call you to see how you’re doing.

Memorial Services

We offer a memorial service once a year to honor and remember your loved one.

Additional Support

Our team will meet with you in person if you need additional support. We’re just a phone call away during challenging times of the year, like birthdays and anniversaries. We can also provide referrals to other bereavement resources.
