Continuous Hospice Care
Continuous hospice care is provided when a patient is experiencing severe symptoms that require 24/7 care. Our team will provide care in the patient’s home for a minimum of eight hours per day to manage your loved one’s symptoms and provide support for your family. We typically provide continuous hospice care for a limited period of time until the patient’s symptoms are under control and can be managed with routine home care. Continuous care may also be used during a period of crisis or when the patient is nearing the end of their life.
Our Bereavement Services
Losing someone you love is one of the hardest things you can experience. Our hospice team provides up to 13 months of bereavement support to help you through this difficult time. We provide the following bereavement services for our patient’s family members:
Mailings & Phone Calls
Our team will send letters every few months and call you to see how you’re doing.
Memorial Services
We offer a memorial service once a year to honor and remember your loved one.
Additional Support
Our team will meet with you in person if you need additional support. We’re just a phone call away during challenging times of the year, like birthdays and anniversaries. We can also provide referrals to other bereavement resources.
